School Nurse Role

• Review student, parent/guardian and school nurse roles then collaboratively develop plan of care with student and parent/guardian based on state regulations, provider orders and NPS Diabetes Management Plan.         
• Discuss field trip plans with student, parent/guardian and teachers.
• Develop or update 504 plan/Individual Health Care Plan.  
• Work collaboratively with student’s teachers regarding medical information, care plan, as well as information regarding symptoms and treatments of hypoglycemia and hyperglycemia.
• Assess student’s competency in self care throughout school year, update plan of care and competency form as needed.         
• Monitor CGM if shared with nurse.           
• Determine communication guidelines with parent/guardian regarding care and supplies.
• Collaborate with student and parent/ guardian(s) to meet prior to the start of the school year.
• Support student’s development of competency in self care and self advocacy in school.
• Review Diabetes Supply Checklist and request missing items.
